    Urgent Partner Search for SC1-BHC-24-2020

    A Spanish public hospital is looking for partners to submit a proposal to the call SC1-BHC-24-2020: Healthcare interventions for the management of the elderly multimorbid patient.

    This project aims to design, implement and evaluate new care pathways for complex chronic patients with Cardiac Failure (CF), merging together a novel intervention programme that has been recently validated, specific health literacy on CF and the validated CDSMP (Chronic Diseases Self-Management Programme) for improving the self-management competencies in complex elderly patients.

    Public hospitals in the field of cardiology, geriatrics or internal medicine, and ethical experts are sought to develop personalised medicine solutions for the elderly within research cooperation agreements. The current consortium consists of hospitals and a university that will develop the evaluation framework.

    Partners Sought:

    • Public hospitals that would like to collaborate in the design and the implementation of a programme that will be based on a specific intervention designed for elderly multimorbid patients affected by CF, and/or a physical therapy.
    • Ethical experts in General Data Protection Regulation (GDPL)

    Deadlines for the call and expressions of interest are 24 September 2019 and 6 September 2019, respectively.
